Detailed Information for: Adult Foster Home - Addictions and Mental Health

Description: Adult Foster Home (AFH) "means any home licensed by the Department of Human Services (DHS), Addictions and Mental Health Division (AMH) in which residential care is provided to five or fewer adults who are not related to the provider by blood or marriage as described in ORS 443.705 through 443.825. For the purpose of these rules, if an adult family member receives care, he/she that advertises, including word-of-mouth advertising, to provide room, board, and care and services for adults, is deemed to be an AFH. For the purposes of these rules, an AFH does not include facilities referenced in ORS 443.715 (1)(2)(3)(4).

Duration: One Year
Fees - application, exam, etc.:  $20.00 per bed non-refundable fee for each non-relative resident.

Fee Exemptions: None
Examination Information: Prior to licensure, the provider must complete the AMH Basic Training Test. A study guide is available to assist with the test.
Continuing Ed Requirements: Annual Training Hours. The AMH will require a minimum of twelve hours of training annually directly related to the care and services for persons with mental illness. Training will include:
(a) Understanding and recognizing severe and persistent mental illness
(b) Mandatory abuse reporting
(c) Medication management, dispensing and documentation
(d) Incident report writing
(e) Resodent rights
(f) Adult Foster Home emergency planning
(g) Fire safety
(h) Complaints and grievances
(i) Cardiopulmonary Resuscitation (CPR)and First Aid
Bonding Requirements: None
Insurance Requirements: None
Prerequisites: A licensee must:

  • Must be at least 21 years of age;

  • Live in a home which is to be licensed unless a residential manager lives in the home;

  • Provide evidence satisfactory to the AMH regarding experience, training, knowledge, interest and concern in providing care to persons with severe and persistent mental illness.


In addition to possessing the characteristics, applicants must meet the qualifications for the desired license classification:

  • Class I - complete Basic Training Course and examination

Renewals: Prerequisites:

  • Applicants and facility are in compliance with licensing rules;

  • AMH has completed an inspection

  • AMH has a current Criminal History Check on applicants and any person (other than resident) 16 years or older.


Required Documentation:

  • Renewal application fee;

  • Correction of deficiencies.

Required Documentation:

  • Application and fee of $20.00 per bed requested for license.

  • A separate application is required for each location operated as an AFH.

  • A written statement for a Licensed Medical Practitioner regarding the mental and physical ability of the applicant to provide care to residents and to operate the AFH.

  • A completed financial information form provided by AMH.

  • A signed letter of support from the Local Mental Health Authority or designee for the applicant to be licensed to operate the adult foster home.

  • A copy of the documentation oof Criminal History Check approval in accordance with OAR 407-007-0200 through 0380 for the provider(s), resident manager, caregiver(s) volunteers and other occupants 16 years of age or older.

  • A floor plan of the adult foster home.

  • A completed Adult Foster Home Self-Inspection Guide.
